Army report blames systemic failures, two broken radio transmissions for 2025 DC crash that killed 67
The Black Hawk and American Airlines collision near Reagan National; the report names no responsible individuals.
The Army finished the report in May and released it Thursday after months of pressure from victims' families, who say the partly redacted report falls short because it does not name who was responsible.
